# Add form
This is a guide for stores that need more than one contact form.
## 1. Add a form manually
In app backend, go to **Manage Forms** -> Click the **Add Form** button in the upper right corner
The app will automatically create a form with a default field.
The form is automatically generated with a single field.
Next, you need to set up everything necessary for the form following these instructions:
## 2. Add a form with AI
In app backend, go to **Manage Forms** -> Click the **Creat with AI** button in the upper right corner
A popup will appear allowing you to type a prompt to communicate with the AI. You can also use some of the pre-listed options: Contact form, Wholesale application, Quote request, Warranty claim, Feedback.
After entering the prompt to create the form, you will see a preview listing the form fields generated by the AI. If it meets your needs, click the "**Use this form**" button; otherwise, continue entering the prompt to have the AI modify the form for you.
After clicking the "Use this form" button, the application will create a form with the corresponding fields that you describe to the AI.
This form is not ready to use yet, you need to set up everything necessary for the form following these instructions:
